That's because that's not the entire quote. It actually states,
“a samurai will use a toothpick even though he has not eaten. Inside the skin of a dog, outside the hide of a tiger”.
A clean outward appearance was a symbol of the samurai’s discipline. They had routines upon awakening that were to be followed with the utmost dedication. The toothpick comment means that no matter what the case or situation, always follow that routine, whether inside the skin of a dog, or outside the hide of a tiger.
